Insurance: I am an in-network provider with Anthem, United Health Care/Optum, Cigna, Aetna, and Sentara. If you wish to use your insurance, I encourage you to contact your plan to find out about your co-pay, co-insurance, or deductible.

If you have an insurance plan other than those listed above and wish to use your out-of network benefits, I will be happy to assist you in filing your claims. You will be responsible for the full fee for the session; your insurance company may reimburse you for part of this payment. Out-of-network benefits vary greatly by plan, so I encourage you to contact your insurance company for further information.

Fees: If you are not using insurance or are using out-of-network benefits, my rates are $175 for the initial consultation and $135 for all follow-up appointments. I accept cash, check, most major credit cards, health savings accounts, and flexible spending accounts.

Cancellation policy: I require 24-hours notice if you need to cancel or reschedule an appointment. If you do not provide 24-hours notice or do not show up for your appointment, you will be charged $80; this cost is not covered by insurance.
